Thank you for considering The House of Promise as your safe place to heal. Below is a link to our Referral Form. This will give us some basic information to begin the intake process.

Please complete the entire Referral Form and include a phone number or email that can be reached during business hours (M-F 8am to 4pm).

Once submitted, please allow up to 2 business days for contact to be made to schedule an intake discovery call. This call typically takes 30-60 minutes. Let us know if your time is limited. Feel free to ask any questions you may have during your intake discovery call.
